Affidavit of Loss LTO format

I have been searching for an "Affidavit of Loss LTO format." I can't find a sample that comes from the said government institution. However, I carefully check these two files here so that it can help the others who have the same issue with me.

First of all, why do we need this affidavit of loss Lto format? Well, we need this most especially if we lost our driver's license. Not only that, you can also modify this sample if let's say you lost your OR/CR or a document that LTO has concern with.
