This is Deped OVAP requirements and PEAC Application application form 2020 file. As a rule, incoming SHS students who wish for a financial assistance need this file.
Anyway, aside from this OVAP application form 2019, there are other DepEd OVAP requirements that you need to prepare.
I. SHS DepEd PEAC Requirements
- You need to have a recent 2x2 colored ID picture on the application form.
- There should be a Proof of financial capacity of the parents, guardian, or any other person who is helping the student. This can be the copy of Latest Income Tax Return.
- Certification of Financial Assistance received from the school, if applicable.
- Certificate of Eligibility (ALS A&E Test/PEPT Result) to enroll in Grade 11.
In addition, you need to remember the following:
II. Guidelines for Submission
- Submit a complete application and supporting documents.
- PEAC National Secretariat should receive the application on the specified date.
- Do not submit multiple applications. If you have applied online, there is no need to fill up this form or vice versa. After all, multiple submissions significantly delay the processing of your application and its results.
